It is essential that the fast-track process be sensitively and thoughtfully communicated to both the individual and, if applicable, their advocate to empower their involvement and ensure their perspectives are valued. A key priority must be to provide the necessary assistance to the relevant person in their chosen location as promptly as feasible, bypassing the full eligibility assessment for NHS continuing healthcare where feasible. Including a summary of the individual’s current presentation and expected condition trajectory—including potential declines, their timing, and mechanisms—within the fast-track pathway tool would be beneficial. The designated ‘appropriate clinician’ must possess expertise in the individual’s health requirements, diagnosis, care, or treatment, and be capable of justifying their alignment with the fast-track criteria through assessment.
It is also important for the ICB to know what the individual or their representative have been advised about their condition and prognosis, and how they have been involved in agreeing the end of life care pathway. This care plan should be provided with the fast-track documentation, or as soon as practicable thereafter, in order for an ICB to commission appropriate care. While the completed fast-track pathway tool itself is sufficient to demonstrate eligibility, a care plan will be required that describes the immediate needs to be met and the patient’s preferences.

According to Guz’s findings in the *ASHRAE Journal*, the San Antonio Public Library generates over 1,400 gallons (5,300 liters) of condensate daily—equating to a gallon per minute—which is repurposed for irrigation.
So even a perfectly administered property tax can be regressive, but that assumes the homes themselves are being taxed proportionally.

Health workforce policy and management
The individual should be fully informed and empowered to participate actively in the assessment process and any subsequent reviews, and their views should be considered. Clinicians completing the fast-track pathway tool should make the individual aware that it will be important to review the individual’s care needs and the effectiveness of the care arrangements. The setting where an individual wishes to be supported as they approach the end of their life may be different to their current arrangements (for example, even though they are currently in a care home setting they may wish to be supported in their family environment). It is the responsibility of the appropriate clinician to base their decision on the facts of the individual’s case and healthcare needs at the time. An individual may at the time of consideration be demonstrating few symptoms yet the nature of the condition is such that it is clear that rapid deterioration is to be expected in the near future. The completed fast-track pathway tool should be supported by a prognosis, where available.

Ecological wastewater treatment provides another option for more comprehensive onsite treatment of both graywater and blackwater. This treatment system consists of a hybrid anaerobic/aerobic treatment and filtration process and a peracetic acid and UV advanced oxidation disinfection process. An advanced, multistep, biological treatment and micro-filtration process treats 100% of the wastewater produced in the 28-story, 293-unit, LEED Gold apartment building. Along with concentrating minerals, cooling towers also concentrate bacteria and other contaminants, including Legionnella (a bacteria that causes potentially fatal Legionnaires’ disease). Water is also intentionally drawn off—a process referred to as blow- down—because minerals and other contaminants become more concentrated as a result of evaporation. In the San Antonio climate during peak summertime months, this translates into roughly 0.5–0.6 gallons per hour for every 1,000 ft2 of cooled area (20–24 l/hr per 1,000 m2).

The Tampa Bay Seawater Desalination Plant, the largest in the U.S., boasts a 25 million gallon (95 million l) per day production capacity. Each day 70 million gallons (260 million l) of highly treated wastewater are injected into an underground aquifer, from which drinking water is withdrawn for 2.3 million Californians in 20 cities. The Colorado River, which provides drinking water for San Diego, Los Angeles, Phoenix, Las Vegas, and many other municipalities, has more than 450 wastewater discharge permits along its course. But some suggest that direct potable water reuse isn’t that different from what we’re doing today.
If an individual meets the criteria for the use of the fast-track pathway tool then the tool should be completed, even if an individual is already receiving a care package (other than one already fully funded by the NHS) that could still meet their needs. This includes where such support is required for individuals who are already in their own home or are in a care home and wish to remain there. The fast-track pathway tool must only be used when the individual has a rapidly deteriorating condition and may be entering a terminal phase. They should contact the appropriate clinician who is responsible for the diagnosis, care or treatment of the individual and ask for consideration to be given to completion of the fast-track pathway tool. A) responsible for the diagnosis, treatment or care of the individual under the 2006 Act the National Health Service (Consequential Provisions) Act 2006] in respect of whom a fast-track pathway tool is being completed; and b) a registered nurse or a registered medical practitioner. Therefore, the completed fast-track pathway tool – which clearly evidences that the person has a primary health need arising from a rapidly deteriorating condition and the condition may be entering a terminal phase – is in itself sufficient to establish eligibility.
